I, , give consent for my child to be photographed for the following purposes

Still photos to be place in center brochures, flyers,postcards, advertisement, as well as, bulletin boards at the center, our parent communication app(Brightwheel) and on company website.

I understand that it is my responsibility to update this form in the event that i no longer wish to authorize one or more of the above uses. I agree that this photo permission will remain in effect for the duration of my child's enrollment at the center.

I further understand that there will be no compensation associated with the use of these photographs on any of the center's print or online advertisement.
